Clinical, functional and incidence outcomes in EDIPPP

Aims: Indicated prevention was tested for effectiveness in a U.S. nationally representative sample at six sites, at the community, as well as the individual, level. Methods: Among 520 youth aged 12-25 referred by community professionals, 337 met SIPS/SOPS criteria and were assigned to low risk (control condition), clinical high risk or early first episode psychosis (both experimental conditions), in a regression discontinuity design. Controls were those scoring at sum 6 or less on the SIPS Positive Symptom subscale. Participants are assessed over 24 months for positive, negative, disorganized and general symptoms, SCID-IV diagnoses, social and role functioning, substance abuse, family functioning and neurocognitive status. In addition, rates of first hospital admissions for the same age group are determined for 5-7 years prior to the intervention and compared to an experimental period of three years after the intervention started, across 6 sites. Results are compared to an adjacent control catchment area for both periods. Results: Rates of conversion were 3.5% in the low-risk control group vs. 10.2% for the CHR subsample. The other major outcomes and analyses from this study will be completed in May, 2012, after which time final results will be available. Outcomes will be reported for conversion, symptoms, SCID-IV diagnoses, family, social and role functioning and substance abuse. Differences in clinical incidence between control and experimental periods and catchment areas for the full sample and the mean change in incidence will be reported. Discussion: The EDIPPP national outcome study will substantially add to knowledge regarding prevention of psychosis.
